It's an old-ish joke that obama's gay (that's half of the male hooker joke, and the other half being that the hooker would also fuck michelle because obama won't). There was also some joke/rumor that michelle is trans (and was given the moniker "Big Mike"). That's the explanation. Now please stop here.
