See, in humans our mammary glands grow based exclusively on genes and environment (gramma had big titties / daughter had a healthy, normal-sized diet). Maiden mares don't have an udder, and the teats are right next to each other, as well as flat against her belly.
The hormonal changes of foaling 'activate' her teats, causing them to swell into a full udder, though over years they will shrink substantially but never completely - they'll eventually look like they're depicted here, each teat having a distinct bag, not unlike human breasts.
