Bloodstained Fields Incident

Medic Knights Field Report

Patient: Ion-Yggdrasil

Patient found by survey team after the discovery of the assassin group Tarot activity in the area. Patient was suffering from three counted gunshot wounds to the abdomen while mentioning severe chests pains with difficulty breathing. Patient was field stripped to assess injuries and condition as heat rate was falling. No injuries to the chest discovered, three penetrating wounds discovered on patient's abdomen, no injuries discovered on patients groin, sheath or scrotum. Previous scarring on patient's shoulder, left chest wall, upper left abdomen to middle groin and right scrotum observed but not crucial to current injuries.

Connected the EKG to the patient's chest, heart rate was erratic before suddenly the patient fell into cardiac arrest. I started compression while Markaus applied the defibrator, patient's heart failed to respond even with increasing voltages, as a last attempt we decided to go in and do an internal.

Markaus took over chest compressions as I made an incision in the side wall of the patient's chest over top an existing scar from a apparent previous thoracotomy. Patient started minor urination probably from the pressures applied as I gained access to the patient's heart where I started massage measures while Markaus changed the defibrator from external to internal.

Shocks started once again and patient's heart responded to the internal defibration though heart beat was still weak, using a clamp I made sure the defibrator would stay in place against the patients beating heart while we moved on to next steps, patient was in unstable condition and the decision was made to operate on the wounds in the field situation.

I closed the patient's eyes before making an incision from just below the bottom of the sternum to just above the patient's sheath, Markaus inserted the catheter and applied the intravenous, patient's blood was recorded as O- and an antibiotic solution was applied. Patient's abdomen was filled full of blood with heavy bleeding occurring from the incision.

Patient's abdomen was pried open with circular retractor wheel and clamps, performed physical examinations of the patient's internal abdominal organs;
Liver - no signs of damage,
Stomach - no signs of damage
Pancreas - no signs of damage
Spleen - missing, removed at earlier date?
Large Intestine - single penetrating wound
Small Intestine - two penetrating wounds
Bladder - no signs of damage
Examination of the patients abdominal organs revealed the wounds were isolated to the bowels, specifically one bulled found in the large intestine and two bullets found in the small, we exhumed the affected sections and were successful in retrieving all penetrating foreign objects from inside.

Once foreign objects were successfully removed, repair of the damaged intestinal track commenced, patient started waking as the final stitching were being placed, which we had to comfort due to elevated heart rate, patient was still mentioning chest pains beyond the thoracotomy which became concerning, possible suffering from some sort of cardiac episode or a form of heart attack.

Once we got the patient calmed we started closing up the abdominal incision and wounds while keeping the patient's mental focus up. Patient was still considered unstable and we feared he could suffer cardiac arrest again if not too careful.

Once abdominal incision and wounds were closed one final external examination was undertook to make sure of no oversight. Patient's vitals remained steady though patient's heart was still of great concern, decided to keep the thoracotomy open for now until patient could be examined by a cardiac physician. While still a bit unstable it was decided to transfer patient to nearest field hospital for further examination and treatment, remainder of patient's clothing and equipment were removed for the transfer which was successful. Follow-up information received was patient underwent a bypass graft procedure at the field hospital as well as a few other surgeries pertaining to abdominal and groin issues from injuries sustained, for further information beyond please see reports documented by the field hospital attendants.
