The cross going through the emblem on the same page where other emblems are visible... is probably intentional.

I believe the discrepancies are just a matter of minor artistic mistakes. The Commander's crest was changing appearance even before Wolfe escaped her bindings.

He gets to say "I told you so". I don't think that's the demon somehow mimicking the commander's crest. She's pretty bad at facial expressions. She might try to sneak out by pretending to be the one guard she killed though.
